The golden rules

• Never overlap. Two GLP-1 medicines at once multiply side effects without doubling benefit.
• Start lower than the conversion suggests. Tolerance does not transfer perfectly between molecules. Moving from Wegovy 2.4 mg does not mean starting Mounjaro at 10 mg.
• Time the switch at a dose boundary. The cleanest moment is when a pen or pack runs out.
• Bring your history. Your previous doses, side effects and results shape the new starting point.

Switching to Mounjaro

From Wegovy or a tablet, everyone starts Mounjaro at 2.5 mg, even from high semaglutide doses. The GIP component is new to your body and needs its own adjustment month. Most people then titrate faster than a brand-new patient, stepping up every 4 weeks until appetite control matches what they had before.

Switching to Wegovy

From Mounjaro, prescribers usually start Wegovy at 0.5 mg or 1 mg depending on your previous dose and tolerance, not the full 0.25 mg restart, but not a direct conversion either. From Wegovy tablets to the pen (or back), the molecule is identical, so the mapping is closer to one-to-one.

Switching to daily tablets

A common maintenance move. From a weekly pen, you start the tablet the day after your next injection would have been due, at a low dose, stepping up monthly. The daily ritual replaces the weekly one with no gap in coverage.

Already on treatment elsewhere? Bring proof of your current dose, a photo of your pen or pack, or a pharmacy record. We continue your titration rather than restarting it, subject to the prescriber's review of your history.
