SURMOUNT-1 recorded an average loss of up to 21% of body weight over 72 weeks. Here is what that looks like as a real timeline, including the slow start nobody posts about.
Month 1: the quiet month
On 2.5 mg, most people notice appetite changes but modest scale movement, often 2 to 4% of body weight, sometimes less. This month exists to build tolerance. People who quit here quit before the medicine has started working.
Months 2 to 3: momentum
Stepping through 5 mg and 7.5 mg, weekly loss of 0.5 to 1 kg becomes typical. Clothes fit differently before the mirror shows it. This is also when protein habits matter most, the weight coming off includes muscle unless you defend it.
Months 4 to 6: the steep middle
The fastest phase. Most responders pass 10% of starting weight somewhere here. Expect one or two plateaus, brief stalls are part of the pattern, and our plateau guide covers what moves them.
Months 7 to 12: approaching the new normal
Loss slows as your body finds its new equilibrium. In SURMOUNT-1, the 15 mg arm averaged about 21% at week 72. The 10 mg arm about 19.5%. 5 mg about 15%. Around a third of people on the highest dose lost 25% or more.
Then the more important question arrives: how to keep it. That is what the maintenance phase is for.
Why results vary so much
Averages conceal a wide spread. Sleep, protein intake, movement, stress, other medicines, and plain biology all move the number. The fairest expectation: most people who stay on treatment and work the habits lose between 10 and 20% over a year. Your prescriber tracks your curve against this at every check-in.
